The repair
The scarred segment is removed and the healthy bile ducts at the liver hilum are joined to a 60–70 cm limb of small intestine (Roux-en-Y hepaticojejunostomy) using fine sutures under magnification — the same anastomosis performed in liver transplantation. High injuries involving the right and left ducts separately are repaired with multiple anastomoses.

Benign strictures from chronic pancreatitis, recurrent stones (hepatolithiasis), choledochal cysts (removed completely because of cancer risk), and malignant strictures from cholangiocarcinoma or gallbladder cancer are all treated with resection and reconstruction.

Can a bile duct injury be treated with a stent alone?
Minor leaks and short partial strictures often respond to ERCP stenting. Complete transections and long or high strictures need surgical repair — repeated stenting only delays it and adds infection.
